I saw "WOLF" as soon as I looked at the thumbnail, even without the other words as context.

I think you can stop at the word "patient", and realize they're talking about lip and throat cancer.

It should be read: "A gentleman is just a [lung cancer] patient."

The smoke looking like letters is just a coincidence.
